Referral Services

1. Assist incoming patients with registration, check-in, and examination procedures.
2. Assist in the examination of referred patients.
3. Return examination reports and referral forms to the original referring institution and physician.

Referral Process

Incoming

1. The referral center receives a referral form (in duplicate) issued by another institution
2. After confirming the referral form, assist with registration and note the medical record number on the referral form
3. Assist patients in visiting the outpatient clinic
4. After the consultation, collect the referral form (returned at the outpatient clinic or collected from the mailbox)
5. Report the patient's subsequent treatment status to other institutions (outpatient treatment process, admission summary, discharge summary, etc.)

Outgoing

1. The physician issues a triplicate referral form
2. The patient goes to the pricing counter to pay
3. The patient goes to the referral center for assistance with contact
4. 1. Assist the patient in contacting the referring institution & registration
   2. The third copy of the triplicate referral form (white copy) must be retained
4. Track the subsequent situation of the referred patient
5. 1. Record report tracking
   2. After receiving a reply from the referring institution, report the patient's subsequent treatment status to the original referring physician
